The Substance Abuse Case Manager at Our Substance Abuse Facility is responsible for assisting and supporting residents through the entire treatment experience, from admission to discharge. Position: SUBSTANCE ABUSE CASE MANAGER
Job Description and Purpose
The Substance Abuse Case Manager reports directly to the Executive Director. This position is
responsible for assisting and supporting residents through the entire treatment experience from
admission to discharge. The Case Manager monitors the course of treatment in a proactive manner to
minimize the barriers that might compromise the treatment and recovery processes. The Case Manager
is responsible for contributing insightful, practical, and meaningful information to the treatment
planning process, through running groups and individual assessments. The Case Manager orientates
each resident to the program rules, expectations, intent, and routine of the daily treatment schedule. The
Case Manager also assists the client with creating an individualized treatment plan that will outline the
client’s goals and treatment objectives.
Major Tasks, Duties and Responsibilities
o Able to present didactic/psychoeducational groups and workshops
o Provide weekly individual sessions
o Develops the Comprehensive Treatment Plan with the input of the client and through
participation in treatment planning meetings and process
o Contacts residents’ family members to set appointments or educate the family on the
residents’ current status and/or progress
o Responsible for reading the shift log at beginning of shift and documenting all necessary
information at the end of the shift
o Documentation of all activities and daily progress of client according to charting
standards within a specific timeframe
o Crisis Intervention
o Assist, teach and/or supervise clients with general program activities
o Conduct audits to ensure that compliance procedures are being followed
Skills, Knowledge and Competencies Required
o Knowledge of drug and alcohol dependency, mental health, family systems and various
recovery resources in the community such as Smart Recovery/other resources in the
community
o Fully understands and maintains policies regarding professional ethics, including
appropriate boundaries and patient confidentiality
o Basic knowledge of referrals, both in and out of the program
o Ability to communicate clearly, with clients, staff, peers, supervisors and non-Hillside
Mission resources.
o Must be able to demonstrate leadership abilities and be able to work with and
communicate with persons from all educational and cultural backgrounds
o The Case Manager must demonstrate tact, initiative, resourcefulness, professionalism,
enthusiasm, and a non-judgmental attitude
Minimum Qualifications

_____ Must be at least 20 years of age
_____ Registered/Certified Drug & Alcohol Technician/Counselor
_____ Class “C” Driver’s License Required
_____ CPR/First Aid certification required, obtain upon employment and maintain current thereafter.
_____Ability to pass pre-employment requirements: Criminal Background Clearance, Negative Drug Screen and provide required documents.
_____ Health Screen required
_____ Negative TB Test Results required
_____ Develop computer skills adequate to perform word processing duties upon employment.
Vision, hearing, manual dexterity and hand-eye coordination must be adequate for performance of job
duties. Able to sit at desk, use keyboard, write and physically perform other job duties. Able to move
about the facility to observe clients and staff.
