Certified Nursing Assistant - CNA

Liberty Behavioral & Community Services, Inc., a leading provider of services for individuals with autism and developmental disabilities, is dedicated to fostering independence, inclusion, and personal growth for our participants. We are currently seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our dedicated team. The CNA will play a crucial role in providing high-quality direct patient care, ensuring that patients receive comprehensive and personalized care.

The Certified Nursing Assistant (CNA) will deliver direct patient care to individuals with developmental disabilities, coordinate with healthcare professionals, and assist in the development and implementation of individualized care plans. This role requires experience in providing patient care to adults with developmental disabilities and the ability to work collaboratively within a multidisciplinary team.

Working locations: Pasadena, Burbank, Glendale, Koreatown, Downtown Los Angeles, Lancaster, Palmdale

Direct Patient Care:

- Assisting with daily living activities: Helping patients with bathing, grooming, dressing, and other personal hygiene tasks.

- Feeding and Hydration: Assisting patients with eating and ensuring they stay hydrated.

- Medication Assistance: Ensuring patients take their medications as prescribed.

- Monitoring vital signs: Checking and recording patient's vital signs such as blood pressure, temperature, and pulse.

- Patient mobility: Helping patients move around, including transferring them from beds to wheelchairs and vice versa.

- Maintaining patient rooms: Keeping patient rooms clean and stocked with necessary supplies.

- Reporting: Observing and reporting any changes in patients' conditions to the nursing staff.

Care Coordination:

- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.

- Coordinate and oversee medical appointments, follow-ups, and treatments.

- Ensure timely and accurate documentation of medical records and care provided.

Health Education and Support:

- Educate participants and their families about health management, medications, and self-care techniques.

- Provide emotional support and counseling to participants and their families as needed.

Compliance and Safety:

- Ensure all healthcare practices comply with local, state, and federal regulations.

- Maintain a clean, safe, and organized environment for participants.

- Adhere to infection control protocols and safety standards.

Requirements

- Completion of a CNA training program

- Minimum of 3 years of CNA experience, preferably with individuals with developmental disabilities.

- Current CPR certification.

- Strong understanding of basic patient care, including hygiene, feeding, and mobility assistance

- Ability to measure and record vital signs such as blood pressure, temperature, and pulse.

- Knowledge of how to assist patients with daily living activities and ensure their comfort and safety.

- Familiarity with common medical terms.

- Excellent interpersonal, communication, and organizational skills.

- Ability to work effectively in a team environment.

- Must have reliable transportation and valid drivers license and insurance

- Must be willing to travel to provide patient care
