Licensed Vocational Nurse LVN

GENERAL SUMMARY:

Friendly Nurses Home Health Care is currently seeking Part-Time Licensed Vocational Nurses (LVNs) to join our team. As an LVN at Friendly Nurses, you will administer skilled nursing care to patients in their place of residence, following physician orders and the plan of care under the supervision of the Director of Patient Care Services.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Provide services in accordance with the plan of care.
• Perform initial evaluation visits and regularly reevaluate patients' nursing needs.
• Initiate and revise the plan of care as necessary.
• Deliver specialized nursing services requiring advanced skills.
• Prepare clinical and progress notes for each patient visit.
• Counsel patients and their families in meeting nursing and related needs.
• Process orders and communicate patient needs and condition changes to physicians.
• Complete certification/recertification orders and discharge summaries.
• Assess and determine the amount and type of nursing care needed for each patient.
• Submit timely and accurate documentation, including OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant changes in condition.

EDUCATION, EXPERIENCE, and LICENSES:

• Graduation from an accredited school of professional nursing.
• Current Licensed Vocational Nurse (LVN) license in the State of California.
• Current Basic Life Support (BLS) Certification.
• Valid Driver's License, active Auto Insurance, and Reliable Transportation.
