POSITION SUMMARY
The In-Home Assessment Nurse Practitioner (IHA NP) is responsible for assessing patient/family psychosocial needs, planning, implementing, procuring services for, and evaluating care needs while meeting with members in their homes. As an IHA NP, your goal is to help improve the quality of life and health literacy by completing an in-home visit (that can last up to one hour) evaluating health status, medical history, current medications, social determinants, and other risks. The IHA NP helps to coordinate, monitor, and ensure information is shared in an appropriate and timely primary, across the continuum of care. The IHA NP will promote the effective utilization of tools and monitor healthcare resources while maintaining a full-time schedule. This position will require travel in your specific area and the surrounding county.
