Position Overview:

As a Care Advocate, you will play a critical role in providing full-service care coordination for our members. You’ll serve as a compassionate and knowledgeable guide, helping members navigate their healthcare journey—from understanding the program services to scheduling appointments, coordinating travel, and facilitating follow-up care.


Key Responsibilities:

  • Manage end-to-end care coordination for members.
  • Educate members on programs.
  • Assist members in provider selection and procedure scheduling.
  • Confirm appointments with patients and provider offices.
  • Maintain accurate member records in CRM systems.
  • Help arrange travel accommodations (e.g., hotel, flights, car rentals).
  • Coordinate follow-up appointments post-procedure.
  • Contact provider offices to request medical records, verify insurance, and schedule services.
  • Maintain regular, guideline-driven communication and follow-up with members.

Must-Have Qualifications:

  • Bachelor's degree.
  • Strong customer service skills.

Nice-to-Have Qualifications:

  • Experience working in a healthcare setting.
