Position Overview:
As a Care Advocate, you will play a critical role in providing full-service care coordination for our members. You’ll serve as a compassionate and knowledgeable guide, helping members navigate their healthcare journey—from understanding the program services to scheduling appointments, coordinating travel, and facilitating follow-up care.
Key Responsibilities:
- Manage end-to-end care coordination for members.
- Educate members on programs.
- Assist members in provider selection and procedure scheduling.
- Confirm appointments with patients and provider offices.
- Maintain accurate member records in CRM systems.
- Help arrange travel accommodations (e.g., hotel, flights, car rentals).
- Coordinate follow-up appointments post-procedure.
- Contact provider offices to request medical records, verify insurance, and schedule services.
- Maintain regular, guideline-driven communication and follow-up with members.
Must-Have Qualifications:
- Bachelor's degree.
- Strong customer service skills.
