As a Home Health Speech-Language Pathologist, you’ll play a vital role in patients’ recovery, helping them regain essential speech and language abilities. By collaborating with healthcare professionals, you’ll ensure that each patient receives the highest quality, holistic care.

• In-Home Assessments: Conduct assessments in the patient’s home and tailor your approach to address their specific needs.
• Treatment Planning: Develop personalized treatment plans to target speech and language goals.
• Therapeutic Interventions: Use specialized techniques and tools to guide progress in speech and language.
• Team Collaboration: Work with referring physicians and the healthcare team to build a coordinated care plan.
• Documentation: Track patient progress and adjust treatment plans to ensure the best outcomes.

• Education: Master’s degree in Speech-Language Pathology and ASHA certification (CCC-SLP).
• Licensure: Active Arizona SLP license with at least one year of clinical experience.
• Communication Skills: Strong ability to communicate effectively and a commitment to providing patient-centered care.
• Certification: Current CPR certification.
• Transportation: Reliable, insured vehicle for home visits.
