**Job Title: Home Health • Occupational Therapist • Days**




**Job Description:**

Join our client's team as a Home Health Occupational Therapist in the vibrant region of Concord, CA. This role offers a 13•week contract with the potential for extension, providing an opportunity to make a meaningful impact on the geriatric population.




**Key Responsibilities:**

• Deliver occupational therapy services to home health patients.

• Manage a caseload of approximately 6 patients per day, with scheduling support provided.

• Collaborate with RNs and other staff for comprehensive patient care.

• Utilize electronic medical records (EMR) systems, with training provided for HCHB.




**Schedule:**

• Monday to Friday, 8:00 AM to 5:30 PM (visits typically start between 9:00 AM and 10:00 AM).

• No on•call or holiday requirements.




**Qualifications:**

• Valid CA Registered Occupational Therapist License.

• Basic Life Support (BLS) certification.

• Minimum of 1 year of home health experience.

• Experience with Oasis is beneficial but not required.




**Additional Information:**

• Scrub color: Any color.

• Orientation includes HR training, HCHB training, and mentor/shadow sessions.
