Role Summary:
This opportunity is ideal for a licensed PT who is passionate about supporting elderly and disabled patients in regaining mobility, independence, and quality of life. This position blends direct clinical care with organizational leadership, making it perfect for those looking to expand their impact beyond traditional therapy settings. In this dynamic position, you’ll support the Rehabilitation Manager and play a critical role in staff development, compliance, operational excellence, and patient care outcomes across multiple care centers. Duties & Responsibilities:
Provide high-quality physical therapy services that help restore function, improve mobility, relieve pain, and promote overall health.
Evaluate patients with mobility impairments and develop personalized care plans aimed at restoring functional independence.
Deliver therapy coverage, either remotely or in-person, for multiple care centers to support staffing and compliance needs.
Participate in assessments, develop care plans, and attend team coordination meetings as part of an integrated patient care model.
Facilitate staff onboarding, skills training, and re-training to ensure consistent service excellence.
Support rehabilitation operations by contributing to data collection, analysis, compliance documentation, and reporting.
Prepare and lead presentations, assist in program planning, and take part in quality improvement (CQI) initiatives.
Join interdisciplinary committees to help enhance patient health outcomes and the delivery of rehabilitation services.
Collaborate with quality teams, department leads, and other healthcare professionals to align services with organizational goals and deliver exceptional patient care.
Other duties as assigned
Required Qualifications:
Active Physical Therapist license issued by the State of California Physical Therapy Board.
Minimum of 1 year of experience working with elderly (geriatric) or disabled populations.
Current Basic Life Support (BLS) certification.
Desired Qualifications:
Bilingual proficiency in English and Spanish, Cantonese, or Mandarin is highly valued.
