Description

Care Manager BSW to join our Care Management department at Providence Little Company Of Mary Torrance Hospital in Torrance, CA. This is a full-time position that will work 8-hour day shifts.

Provides direct services to meet the psychosocial needs of patients and their families, to include: Middle and older adults at TCC/TCU. Provides support to the patient care areas.

Required Qualifications:

• Bachelor's Degree
• 1 year recent experience.

Preferred Qualifications:

• Discharge planning.
