Role Description

This is a full-time remote role for a Home Health Physical Therapist. The Physical Therapist will be responsible for evaluating patients' physical conditions, developing individualized treatment plans, and providing therapeutic interventions. Responsibilities include coordinating home care services, educating patients and their families, and documenting patient progress. The role requires collaborating with other healthcare professionals to ensure optimal patient outcomes.

Qualifications

• Skills in Physical Therapy and Rehabilitation
• Experience in Patient Care and Home Care
• Knowledge of Medicine related to physical therapy
• Strong communication and interpersonal skills
• Ability to work independently and remotely
• Licensed Physical Therapist in the respective state
• Bachelor's or Master's degree in Physical Therapy
• Experience in home health care is a plus
