Position Summary:
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to provide in-home nursing care to patients under the supervision of a Registered Nurse (RN). The ideal candidate has strong clinical and interpersonal skills and is committed to supporting patients' health, independence, and well-being.

Key Responsibilities:

• Provide direct patient care in the home according to the established plan of care.
  
  
• Perform skilled nursing procedures such as wound care, medication administration, injections, and catheter care.
  
  
• Monitor and document patients' condition and report any changes to the supervising RN or physician.
  
  
• Educate patients and family members on disease process, medications, and care needs.
  
  
• Collaborate with the interdisciplinary team to support the patients recovery and goals.
  
  
• Maintain accurate, timely, and thorough documentation in compliance with state, federal, and agency guidelines.
  
  
• Follow infection control and safety protocols at all times.
  
  

Qualifications:

• Current and valid LVN license in the state of [Insert State].
  
  
• Current CPR/BLS certification.
