We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. *Responsibilities*
* Administer medications and treatments as prescribed by physicians.
* Monitor patient vital signs and report any changes to the healthcare team.
* Assist in diagnostic evaluations and discharge planning for patients.
* Provide care in specialized areas such as memory care, critical care, and occupational health.
* Perform specimen collection processing and phlebotomy as needed.
* Document patient information accurately using electronic health record systems such as Athenahealth and eClinicalWorks.
* Educate patients and their families about health conditions, treatment plans, and preventive care.
* Collaborate with interdisciplinary teams to ensure comprehensive patient care.
*Skills*
* Proficiency in using electronic health record systems, particularly Athenahealth and eClinicalWorks.
* Strong understanding of critical care principles and practices.
* Experience in diagnostic evaluation and discharge planning processes.
* Knowledge of memory care techniques and occupational health standards.
* Familiarity with ophthalmology procedures is a plus.
* Ability to perform specimen collection processing efficiently.
* Excellent communication skills with a focus on patient education and support.
* Strong organizational skills with attention to detail in documentation and patient care.
