Job Title: Licensed Vocational Nurse (LVN) • Home Health, Day Shift
Job Description:
Our client is seeking a dedicated and experienced Licensed Vocational Nurse (LVN) for a 13•week contract in home health care. The ideal candidate will have a flexible schedule, with the ability to work 40 hours per week. Weekend availability may be required.
Job Responsibilities:
• Provide high•quality patient care in a home health setting.
• Manage a caseload of approximately 20•25 patients per week.
• Complete all documentation on the same day of service.
• Attend mandatory in•person meetings at our client's location once a week.
Job Requirements:
• Must hold a valid CA LVN License.
• Minimum of 2 years of experience in home health care.
• Proficiency in OASIS documentation is required.
• Experience with Homecare Homebase (Productivity points, minimum average 26•28).
• No more than a 1•year gap in employment.
• BLS Certification.
Additional Information:
• The position is based in Marin County and Sonoma County, CA.
• Scrub color is not specified.
• Personal Protective Equipment (PPE) requirements will be provided.
• Parking details will be provided.
• Pay cycle runs from Monday to Sunday.
• Orientation will last approximately 2•3 days.
• Mileage will be reimbursed at .70/per mile.
