**Responsibilities**
+ Proactive, collaborative team member in a hospice care environment
+ Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
+ Commitment to caring for patients and partners
+ Complete all documentation and records in a timely and accurate manner
+ Follow established policies and procedures as related to the scope of practice
+ Maintain continuity of patient care in a hospice setting
**LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS**
+ Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in state of practice
+ Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care
