**Your Impact:**

As a Home Health RN, you'll be the heart of your patients' care:

+ Lead your caseload with autonomy-assess, plan, and deliver care tailored to each patient

+ Provide compassionate guidance and support to patients and families

+ Coordinate care with doctors, therapists, and caregivers to keep everyone on the same page

+ Adapt plans as patients' needs evolve, ensuring the best outcomes

+ Maintain HIPAA-compliant documentation while being the trusted voice patients rely on

**What We're Looking For:**

+ Active RN license with 1+ years of clinical experience

+ Comfort traveling within the territory (about 50% travel)

+ Reliable transportation, valid driver's license, and auto insurance

+ Home Health or Hospice experience is a plus

+ Associate or Bachelor's in Nursing preferred

+ Experience with HomeCare HomeBase is a bonus
