How you’ll make an impact in this role

Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

• Deliver compassionate, high-quality direct nursing care by expertly implementing and continuously monitoring personalized patient care plans.
• Coordinate multidisciplinary patient care, serving as the central link to ensure seamless collaboration and well-coordinated service strategies across all disciplines.
• Assess and strategically plan the patient’s transition from care by coordinating timely discharge planning to drive safe and successful patient outcomes post-stay.
• Document and clearly communicate changes in patient condition and progress to ensure the entire care team has accurate, timely information for effective decision-making.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
