  • Develops and implements innovative programs with the Fund’s management team to assist beneficiaries in health and daily living.
  • Networks with vendors to develop health fairs and events for beneficiary access and participation.
  • Conducts health education seminars for beneficiaries.
  • Identifies health care issues and implements case conferences with medical director and/or medical management vendor.
  • Develops and maintains effective working relationships with health and medical management staff, and other medical professionals.
  • Prepares written reports and/or oral presentations to trustees and/or operations staff on clinical field service office activities.
  • Records all beneficiary related interactions timely in the Fund’s medical management system.
  • Productively meet the performance measures established by the Funds.

Qualifications

Required

  • Bachelor's degree in nursing from an accredited college/university plus five years’ experience in nursing, or
  • Associate degree in nursing or hospital diploma program plus 5 years home health experience and/or geriatric outpatient clinic, and a certificate in case management recommended.
  • In states where compact state licensure is available, must obtain within one year of employment.
  • Current knowledge of nursing methods and practice, including community health nursing principles.
  • Knowledge of managed care principles, health care management, case management, and disease management.
  • Cognizant of the health care needs of women and geriatric patients.

Preferred

  • Three years in community health nursing

Compensation: $35/hr.

General Description:

Coordinates the delivery of health services that will impact beneficiaries' health, well-being, and quality of life. Implements and manages innovative medical programs that support the medical management principles of the Funds. Identifies, establishes, and maintains relationships with providers and community agencies to promote beneficiary welfare and health education.

Promotes beneficiary access and participation in health promotion, disease prevention, and health education programs.

Duties and Responsibilities:

  • Complete applicable nursing assessments in the homes of beneficiaries to coordinate the health services to improve health, well-being, and quality of life.
  • Collaborates and maintains close working relationships with providers and community agencies, refers beneficiaries in need of services as appropriate.

  • Prepares written reports and/or oral presentations to trustees and/or other operations staff on the status of the activities in the clinical field service office.
  • Records all beneficiary related interactions timely in the Fund’s designated medical management system.
  • Productively meet the performance measures established by the Funds.

Qualifications:

Education and/or Experience:

Bachelor's degree in nursing from an accredited college/university plus five years’ experience in nursing. Three years in community health nursing desirable,

or

Associate degree in nursing or graduation from a hospital diploma program plus 5 years home health experience and/or geriatric outpatient clinic. A certificate in case management recommended.

In states where compact state licensure is available required to obtain within one year of employment.

Technical Business Industry Knowledge:

Current knowledge of nursing methods and practice, including community health nursing principles. Knowledge of managed care principles, health care management, case management and disease management. Also, must be cognizant of the health care needs of women and geriatric patients.
